1.  Log into your patient portal

Visit Artemis RMA Patient Portal (artemisrmaportal.com) to create your account and log into your portal. Each patient will have a separate patient portal.

a. Once you have downloaded the app or visited the ArtemisRMAPortal.com website, tap/click the Sign Up link under the login button.
b. Enter your personal information (name, date of birth, phone number, & email) so that the portal can cross-check our system to find your patient record.
c. Once a match is found, Artemis will email a 6-digit access code to you. This access code is a 2nd factor of authentication to ensure your information is safe and secure.
d. After that you'll be asked to review the Terms & Conditions of the app, and set up a username, password, and a few security questions.

2.   Complete health history questionnaire

Each patient needs to sign into the portal to complete the disclaimers and health questionnaire.  Please note that these questions are not the same as those in our new patient packet.  They are crucial for our doctor and staff as this information will help ensure that your visit goes smoothly and successfully.

You can access the questionnaire by entering the 'Patient Forms' tab and selecting the 'Patient Intake'.

A completed intake form from both partners (if applicable) is necessary for a successful visit. Our team will reach out if we do not have a completed intake form completed prior to your visit.

3.   Any prior, relevant medical records

Females: Please provide records related to any prior fertility treatment and reproduction, including the latest pap smears or pelvic exams, all blood labs, and gynecological.
Males: Please provide any records related to prior fertility treatment, reproduction, and any prior semen analyses.

If you are here for a second opinion from another fertility clinic, please inform us so we can ensure your records release is valid for that clinic.

We have requested your records if you have completed the records release form attached to your registration packet, but it is also your responsibility to ensure that they are faxed or emailed by your provider. It was a pleasure speaking with you today! As a reminder, your appointment is being held until your deposit has been paid. You will receive a call from our office to collect your $100 deposit. 

 

Paulinna Huynh will meet with you ONLINE in your Telehealth room on Tuesday 12/31/2024 at 10:00 AM. Dr. Kathleen Lin and Paulinna Huynh work collaboratively and will be part of your care team!
You should have already received your Zoom log in information in a separate email. 

 

Please arrive 10 minutes before your scheduled appointment for check-in.
 
As we prepare for your appointment, we must receive you and your partner's (if applicable) intake forms and completed consents no later than 3 business days prior to your scheduled appointment. This allows the team time to complete your registration and provide the necessary information to the provider in preparation for your appointment.  If your appointment is scheduled within 3 business days, please submit your forms ASAP. If you are unable to keep your appointment, please contact us no later than 1 business day prior to your scheduled appointment to avoid a $50 non-refundable cancellation/reschedule fee. All co-pays, deductibles and/or coinsurance payments are due at the time of your visit.
 
Both intake forms and consent forms can be completed through our Artemis RMA Portal. Please follow the instructions below to download the portal.
REQUIRED NEXT STEPS FOR YOU AND YOUR PARTNER (if applicable):
 
1.    Download the mobile app by searching "Artemis RMA Portal" in the Apple App Store or Google Play Store. You can also access your portal from a desktop at www.ArtemisRMAPortal.com.
        a.    Please allow Push Notifications so that you can see when you have received a message from your team, get reminders about your upcoming appointments and more.
 
2.    Each of you will register your own account. Log in and see the "Patient Forms" section for your required intake form. You must complete all sections and sign to complete the intakes.
       (In the app this can be found in the more screen, on the desktop version it can be found in the Main Menu bar)
 
3.    Please attach a copy of your government issued ID, insurance, and prescription cards for both you and your partner to the document sent via DocuSign.
 
4.    If you or your partner have had previous fertility testing/treatment please request your medical records from your doctor's office. Previous medical records should include prior treatment, latest pap smears or pelvic exams or any prior semen analyses. They can be faxed to 425-955-4990 or emailed seattle@ivirma.com

We're updating our Terms of Service on May 22, 2024, so we wanted to let you know ahead of time.

These changes won't affect the way you use our services, but they should help make it easier for you to understand what to expect from Google — and what we expect from you — as you use our services.

What do I need to do?

  • If you're under the age required to manage your own Google Account:
    • We sent this email to your parent or guardian so that they can help you understand our updates better.
    • Please discuss this email with your parent or guardian to decide if you need to do anything different with your account.
  • If you're a parent or guardian, and you allow your child to use the services:
    • Please review the updates to our terms with your child and help them decide whether they need to make any changes to their account.
    • Please remember that these terms apply to you and you're responsible for your child's activity on the services.
  • If you're the administrator of a Google Workspace corporate or educational account and you've enabled your users to access Google Additional Services:
    • Our new Terms of Service won't affect the Google Workspace agreement between Google and your organization. These new terms will only apply to those users to whom you've given access to Google Additional Services. You can always manage whether your users have access to Google Additional Services, and which ones, in your Admin console.
  • If you're a Google Workspace reseller whose customers have enabled their users to access Google Additional Services:
    • Our new Terms of Service won't affect your Google Workspace agreement with your customers. These new terms will only apply to your customers' users who've been given access to Google Additional Services. Your customers can always manage whether their users have access to Google Additional Services, and which ones, in their Admin console.
  • If you're any other user of the services:
    • Please read this email to understand our updated terms and your options for further action.
    • If you agree to the new terms, no further action is needed.

  • Generative AI terms. We're moving our existing Generative AI Additional Terms to our main Terms of Service and adding other AI-related clarifications. For example – we won't claim ownership over original content generated by our AI-powered services.
  • More clarity on abusive activity. We're providing more examples and details about abuse and interference with our services that isn't allowed.
  • More details on limitations of liability. For users outside the US, we're adding clarifications to our limitations of liability and indemnity sections to avoid any misunderstandings in light of local laws or customs.
  • More clarity on disputes. We're clarifying that if you violate our terms, our remedies aren't limited to suspension or termination of your access to the services, but may include other remedies under applicable law. If problems or disputes arise between us about these terms, you'll have the opportunity to describe the issues and address them.
  • Updates to reflect how our services work. We're adding language about how our services work, and updating certain Google service brand names that have changed over time.
  • For users based in the European Economic Area (EEA) only:

If you don't agree to the new terms, you should remove your content and stop using the services. You can also end your relationship with us at any time, without penalty, by closing your Google Account.
